Presentation and Outreach Requests
Thank you for your interest in partnering with CAPS! If you would like to request an outreach program or presentation from CAPS, please complete the Outreach Request Form below. This form should be submitted for all requests, including requests for CAPS presentations at new student orientations.
- Please submit this request form at least 3 weeks prior to your preferred program date.
- Please indicate the steps you will take to ensure that at least 10 participants will attend.
- Due to limited staff resources and availability, particularly during periods when CAPS is experiencing peak clinical demands, we may not be able to meet all requests.
- A CAPS staff member will review your submitted request form and will be in contact with you within 5 business days.
PLEASE ENSURE YOU ARE SUBMITTING AN OUTREACH REQUEST FORM FOR THE CORRECT CAMPUS. CHICAGO CAPS IS UNABLE TO COMPLETE OUTREACH REQUESTS FOR EVANSTON EVENTS, AND VICE VERSA.
